An orchiectomy is a medical procedure that removes the testicles. In many cases, for transwomen, the skin from the scrotum is left behind to be used for genital surgery. Many transwomen choose to have the orchiectomy at the same time as genital surgery, but some do not. It’s also an option to have only one procedure done or to have none at all. It all depends on the woman’s preferences.
